Rates
$100 per 50-minute session

Insurance
The only insurance company that I provide billing services for is Coastal TPA, as part of Community Health Plan.  I am not a provider for any other insurance companies, but am happy to provide you with a billing statement that you can present to your insurance company for possible reimbursement.

I suggest that you check with your insurance company about your behavioral health coverage to see whether out-of network providers are covered, and at what rate.  They may specify certain diagnoses that are covered and others that are not.

You may want to ask the following questions:

• Do I have mental health benefits?
• What is my deductible and has it been met?
• How many sessions per calendar year does my plan cover?
• How much does my plan cover for an out-of-network provider?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Payment
Cash and checks are accepted for payment.

Cancellation Policy
24 hours advance notice of cancellation of your appointment is needed, otherwise the full cost of the appointment is charged.
